sounds like perhapsp speech file is getting corrupt (and perhaps other pc issues).  this will happen if you don't shut down dragon properly.  So please ensure that your clients close dragon when done dictating
eg. say "exit naturally speaking" when done dictating or
click on the dragon icon, top left corner and select exit to properly shut down the program.
